Ms. Eva Chan graduated with a Bachelor of Social Work from Hong Kong Polytechnic University and holds a Master’s degree in Gerontology and EOL care from The Chinese University of Hong Kong Medical Faculty. She has over 20 years of experience as a registered social worker, having served in both large and small organizations, including subsidized elderly homes, self-financed day care centers, dementia services, and community care services. In recent years, Eva has actively explored innovative service models, focusing on promoting rural community capital development, rural care services, and enhancing the effectiveness of primary healthcare. She led the team to successfully implement the “Sai Kung Rural Community Telemedicine and Medication Guidance Pilot Program,” which won the 2021 Best Practice Award.  

Eva specializes in service development and social innovation, and she has assisted her organizations in securing multiple funding grants. She also manages the social enterprise “Live Nature,” actively promoting the integration of ecotourism with sustainable community development.
